By clicking submit I hereby irrevocably permit, authorize and license Ultimate Medical Academy, its clients, agents, licensees, affiliates, successors and assigns (herein “UMA”), to display, publicly perform, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, modify, alter, edit, adapt create derivative works, exploit, sell, rent, license, otherwise use and permit others to use my name, image, likeness, appearance, voice, statements, opinions, professional and personal biographical information, and all materials created by or on behalf of UMA that incorporate any of the foregoing (“Publicity Information”), on a perpetual basis throughout the world and in any medium or format whatsoever now existing or hereafter created.
My Publicity Information may be used, for any purpose, in promoting UMA or its affiliates.
Examples include but are not limited to:
I understand that UMA may edit, shorten or alter my Publicity Information for length, formatting, or for any other purpose. I hereby waive any right to inspect to approve the finished video, film, photographs, art, advertising copy, or printed matter that may be produced in conjunction therewith, or the eventual use that it may be applied, and shall be without liability to me for any illusionary effect or unintended distortion.
I agree that UMA has the right to attribute my statements (or statements in different words which have substantially the same meaning) to me, which is an expression of my personal experience and belief. All Publicity Information provided by me is truthful, honest and accurately reflects my actual experiences.
I acknowledge that I am under no obligation to provide this release or any Publicity Information. I warrant that I am providing this information freely and on my own accord. I also understand that I may be contacted by UMA (or its authorized representative) at a later date to provide additional data regarding my experiences with UMA. I agree and acknowledge that no further consent is required and no royalty, payment or other compensation will be paid to me.
In connection with the foregoing, I agree to release, indemnify, and hold UMA harmless (including its officers, directors, employees, and agents) against claims, liabilities, obligations, expenses (including attorney’s fees) or damages arising out of any misrepresentation made in connection with my Publicity Information or from all liability or possible causes of action against UMA, whether in law or equity I may have or may hereafter acquire.
This agreement constitutes the sole, complete and entire agreement regarding the Publicity Information between myself and UMA and its agents, affiliates and assigns, and I am not relying on any other promises or representations whether oral or written. I warrant and represent that I am not restricted by any other commitment or prohibition from entering into this release and authorization, and that I am over 18 years of age, and have read, understood and agree to the foregoing.
